BMMS Outdoor Ed

This form is only to be filled out by a physician, if your child must take medication while at Sandy Hill. A couple of things to keep in mind:

1. If your child will need to take prescription and/or over the counter medicine while on the trip, a physician must complete the attached yellow HCPSS medication form for each separate medication that your child requires.

 

2. Physician medication forms currently on file in the BMMS health room cannot be copied and used for this trip.

3. ALL medications must be turned into the Burleigh Manor health room (between the hours of 8 am and 2:30pm) by SEPTEMBER 7. If this does not work for you, please email us, so we can arrange a drop off.

4. Any medication required during school hours including prescription, “over-the-counter” (OTC) and homeopathic/ herbal medications require a medication order signed by an approved healthcare provider.

5. Over-the-counter (OTC) medication must be in the original, unopened, container The parent/guardian should write the student’s first and last name on the medication container.

6. Only Required Medications can be Administered at Camp.
